No. Having 6/6 eyesight means that you can see standard-sized letters clearly at a distance of 6 metres under the conditions of an eye-chart test.
A person may have 6/6 vision and still have:
- Early glaucoma
- Diabetic retinopathy
- Peripheral retinal tears
- Early age-related macular degeneration
- High eye pressure
- Reduced peripheral vision
- Poor contrast or night vision
- Dry-eye disease
- Problems with near vision or eye coordination
A comprehensive eye examination evaluates much more than the letters you can read on a vision chart.
In this blog, Dr Aman Kumar, a retina and cataract surgeon in Delhi, will answer what 6/6 vision means, what a 6/6 eye test leaves out, and why some of the most serious eye conditions can hide behind perfect vision.
What does eyesight 6/6 mean?
In the term 6/6 vision:
- The first 6 means that you are standing 6 metres from the vision chart.
- The second 6 means that a person with standard visual acuity should also be able to read that line from 6 metres.
Therefore, 6/6 indicates standard distance visual acuity.
It is broadly equivalent to 20/20 vision, which is commonly used in countries that measure distance in feet. However, 6/6 is a measurement of only one aspect of vision: distance sharpness.
Understanding Visual Quality
This test helps patients understand how small a letter they can resolve, in good lighting, from a fixed distance, using both eyes open.
- It does not test how well you see in dim light.
- It does not measure how much contrast you need to make out a shape
- How wide does your side vision reach
- Your eye pressure
- The condition of your retina.
What Can a 6/6 Eye Test Miss?
Eye conditions can affect your vision silently while having perfectly normal-looking vision for years.
- Glaucoma
Glaucoma damages the optic nerve slowly, usually without any pain or warning signs. In India, close to 9 in 10 glaucoma cases are only detected once the disease has advanced because the eye chart fails to detect a problem. - Early Age-Related Macular Degeneration (AMD)
AMD affects the macula. Indian eye studies show early-stage eye conditions becoming more common after age 60, which is exactly why a retinal scan matters as much as a chart reading once you’re past midlife. - Diabetic Retinopathy
High blood sugar affects the sharpness of vision. A nationwide screening study in India found retinopathy in roughly 1 in 8 people with diabetes, many with no visual symptoms initially. - Colour Vision
The Snellen chart does not test colour discrimination. - Near Vision
Distance acuity does not measure how clearly you can read at close range.
After approximately 40 years of age, many people develop presbyopia and need reading glasses despite having 6/6 distance vision.
Eye Conditions That May Exist Despite 6/6 Vision
1. Glaucoma
Glaucoma damages the optic nerve.
2. Early Diabetic Retinopathy
Diabetic retinopathy is caused by damage to the retinal blood vessels.
3. Peripheral Retinal Tear
A small retinal tear in the far peripheral retina may not immediately affect central vision.
4. Early Age-Related Macular Degeneration
Early age-related macular degeneration may produce subtle retinal changes before visual acuity falls substantially.
5. Early Cataract
A small or early cataract may not reduce high-contrast chart vision significantly.
6. Dry-Eye Disease
Dry-eye disease can cause fluctuating vision even when the best measured acuity is 6/6.
Who Should Never Skip Preventive Eye Exams?
Some people carry a higher risk of silent eye disease and need more frequent checks, regardless of how sharp their vision feels:
- With diabetes, since diabetic retinopathy can progress for years without symptoms.
- Over 40, when glaucoma and early AMD risk increases.
- With a family history of glaucoma, AMD, or retinal detachment.
- High myopes (significant short-sightedness), who carry a higher risk of retinal thinning and detachment.
- With high blood pressure, which can quietly damage the retina’s blood vessels.
- Heavy screen users, since prolonged near work is linked to faster myopia progression, particularly in younger patients.
Can You Regain 6/6 Vision After Retina or Cataract Surgery?
- Cataract surgery often restores vision close to 6/6 in eyes with no other retinal disease.
- Retinal detachment surgery aims to reattach the retina and preserve as much vision as possible. Full 6/6 recovery is realistic when the detachment is detected and treated early.
- Delayed treatment decreases the chances. Once the macula has stayed detached for several days.
- The optic nerve is healthy
- Cornea is healthy
Why Retinal Health Matters Before Cataract Surgery
A cataract and a retinal condition can exist at the same time. If the cataract is removed but the retina is unhealthy, vision may not improve as much as expected. Thorough preoperative retinal assessment is important.

How AKIO Treats Retinal Detachment and Cataracts to Restore 6/6 Vision
We treat the retina and the cataract as one, not two separate problems:
- We scan the retina before touching the cataract.
- High-resolution OCT imaging checks retinal health first, so nothing is missed.
- We use 3D micro-incision cataract surgery, with premium IOL options (toric, multifocal, EDOF) matched to your retinal and lifestyle needs.
- We treat retinal tears and detachments with painless laser procedures for minor cases and advanced vitrectomy for detachments.

As a retina hospital in Delhi with a dedicated diagnostic and surgical retina team, AKIO’s eye examination procedure detects what a 6/6 eye test alone cannot.
If you’re looking for the best retina hospital or the best ophthalmologist in Delhi for an overall eye checkup, our team examines the retina and the cataract together.
FAQs
Can diabetic retinopathy exist with 6/6 vision?
Yes. Early diabetic retinopathy commonly causes no noticeable symptoms and may be present while central vision remains normal.
How long does a 6/6 eye test take?
It takes hardly 5 minutes, but a comprehensive eye examination usually takes 30 to 45 minutes.
What is the difference between 6/6 and 20/20 vision?
They describe the same level of visual acuity using different measurement systems.
Can 6/6 vision be attained back from the retina and cataract surgery?
Often, yes, for cataracts and for retinal surgery when treated early, though outcomes depend on how much damage occurred before treatment.
Can someone have a retinal tear and still see 6/6?
Yes. A peripheral retinal tear may not immediately affect central visual acuity.
Can cataract exist with 6/6 eyesight?
Yes. Cataract may cause glare, halos, reduced contrast or poor night vision before it significantly reduces high-contrast chart acuity.
Does a 6/6 test detect retinal detachment?
Not reliably. A peripheral retinal detachment may initially spare central chart vision.A dilated retinal examination is required when warning symptoms are present.
How can I maintain healthy vision?
Attend eye examinations, control diabetes and blood pressure, avoid smoking, wear suitable UV and injury protection, use prescribed glasses and seek care promptly for new symptoms.
